How is the 3D space UCS Icon activated as shown on page 25 of inside-proge cad

The ucs icon is controlled via the ucs command and alternativly by the ucsicon system variable. When the ucs is in the world setting it represents a 2d view of a triangle. When you creat a new orientation and/ or a named ucs using the ucs command you will notice it change into a representation of three dimensions. One thing to remember,  the view your looking at is not necessarily the plane you are drawing in.
